Technical Service Order with Claim Checks

This is a Technical Service Order with claim checks! The form prints with your company logo, name, number and address. Company details print on the top of the form as well as on the bottom claim check.

The form layout is as follows. Beside the company information there is a box that titles this form as a “technical service order”. There are lines for customer name, address and two phone numbers. An original installation date box sits next to the company phone numbers. Above that is another place to record the promise date. This form allows up to three items for servicing. Whether it be appliances, small engine repairs or other technical equipment, technicians will be able to list separate make, model and serial numbers for each one. The small checkbox to the right is to denote the kind of job this is.

Furthermore, the form includes a large box for explaining the services the customer requested. The table at the bottom neatly prices out the parts needed and the cost for each part. The total boxes will help the customer understand the full scope of their request and the final costs. This claim check service order also includes payment information for everyone’s convenience.

There are two perforated claim checks. The first one is an identification card. This card allows the technician to briefly identify the part and the service requested. The bottom tear-off goes with the customer. This will make finding their order and their items easier.
